Before you fill in the forms to register with us please make sure you live within our practice boundary by entering your postcode below.

Register as a new patient

When you register with us you will be allocated a named GP, and offered a new patient health check.

You will be asked to fill out a medical questionnaire. This is because it may take a considerable time for us to receive your medical records. When you come to the surgery you will be asked to sign this form to confirm that the details are correct.

We prefer to register all members of a family at one address to give full family care. 

Our purpose-built premises is fully accessible to people in wheelchairs. We can also see temporary residents.

New students registrations

Registration for University Students in Halls of Residence

It is essential that students register with a local GP surgery when they move into Halls of Residence.
This will provide you with easy access to full medical services nearby during your time at university.

Please remember to add your room number and mobile number to the registration forms.

Temporary patient registrations

If you are ill while away from home or if you are not registered with a doctor you can receive emergency treatment from a local GP practice for 14 days. After 14 days you will need to register as a temporary or permanent patient.

You can be registered as a temporary patient for up to three months. This will allow you to be on the local practice list and still remain a patient of your permanent GP. After three months you will have to re-register as a temporary patient or permanently register with that practice. You cannot register as a temporary patient at a practice in the town or area where you are already registered.
